Notes
1. This certificate can be used for the Household registration and family planning statistics. The correct information requested in this form must be provided to the Midwife by baby’s mother;
2. Before the household registration, the police station has to verify this information against the House Hold book. This certificate will be collected to the same governing level office of Family Planning;
3. A note need to be made in the case of adoption or fosterage;
4. It should be indicated by marking if the birth is within the planning and if midwifery is old or new method.

Leaving Hospital Diagnosis: Intrauterine pregnancy. 35+6 W2/1 ROM. Natural labor. Premature delivery.
Health condition of patient at time of leaving hospital: A female baby was born in natural labor. The baby had a weight 3100g and length 48cm. The mother was in normal condition after delivery. Cutting in the perinea has healed.
Recommendations: Take standard maternity leave after delivery. Mother and baby are supposed to be back the hospital for checking in the day of 45th(Saturday morning).
